VerSprite uses an individual signed consent form to enroll the intended mobile-phone recipient into the VerSprite SMS Alerts program. The form may be provided as a PDF attachment in the client's existing email conversation.
Blank SMS consent form
This is the public example of the complete, unsigned consent form, including the business name, service description, phone number field, optional checkbox, frequency, carrier charges, HELP/STOP instructions, terms, privacy policy, signature, and date.

How written opt-in works
- A VerSprite representative emails the blank consent form to each intended SMS recipient, with the Privacy Policy and Terms & Conditions links.
- Each intended recipient reviews the service disclosure, enters their name and mobile number, checks the SMS consent box (not preselected), signs, and dates the form.
- The recipient replies privately to VerSprite with the completed and signed document. VerSprite stores the completed form in an access-controlled internal record; signed forms and personal phone numbers are not posted on this website.
- Only after VerSprite verifies affirmative consent from the actual recipient is that mobile number added to the SMS alert recipient list.
- Recipients may reply STOP to opt out or HELP for assistance. They may also contact their VerSprite representative or use our company contact page. Revoked or declined consent means no further alerts to that number.
